PMA is contracted with most major insurance carriers and all insurance claims will be submitted to any insurance carrier as a courtesy to the patient. Payment for any insurance co-payments and/or deductibles is expected at the time services are provided.

Please see below a listing of insurances PMA currently is contracted with.  It is the responsibility of the patient or sponsor to know the specific requirements and limitations of the coverage of the insurance plan held. Questions about insurance coverage and insurance payment or non-payment for services rendered should be directed to the insurance carrier to whom claims are submitted.

If you do not have insurance, you will be expected to pay the entire consultation fee at the time of your visit. Any additional fees for medically necessary procedures or testing will be billed to you and payment is required within 30 days.

When necessary, our staff will work closely with patients who require a payment plan.
