Referrals
Patients seeking care for a pulmonary, sleep or infectious disease-related condition from a PMA provider for the first time must be referred by their Primary Care Provider (PCP) or other provider authorized to make such referrals to specialists.
Patients who have not been seen at PMA within the last 3 years, and who wish to be seen by a PMA provider also need a referral from their PCP.
Patients seeking care for allergies/asthma/immunology conditions and who have Medicare or a PPO or other similar insurance arrangements may schedule an appointment at any time with no need for a referral. Patients with managed care or other insurance arrangements may need to contact their primary care provider to request a referral authorization.
If you have any questions about whether PMA is contracted with your insurance plan, please feel free to contact our Patient Services staff at (916) 482-7623.
